I had that behavior too, come to think of it. I never really bothered with looking into that because I rarely change survivors to begin with. But certainly worth noting.

for some reason this mod made the survivors model disappeared when selecting a survivors

For those who don't have changed main menu screen appearing - you have to set random menu background in game settings

Strange, it still works for me. Maybe check that it's enabled in the ingame mods menu?
