She doesn't drop it 100% of the time, otherwise you'll be just swimming in trinkets. But the chances are pretty high. And when she doesn't drop the class trinket, she drops general loot from vanilla loot table of pretty good quality.
As I mentioned before, for many monster mods there are same complaints - statistics doesnt work as intended and 1% chance are not really 1%. Like, I met vestal in almost my every dungeon run. As for other game statistic based interactions and checks I almost sure there are some hidden mechanics which increase/decrease chances of bad events depending of how well player performs.
The chance of encountering Vestal I think is 1% and only in rooms of crypts (no hallways). Jester is only in hallways but across all dungeon.
About permanent defeat - there is no known way to do this even if I wanted to. At least not known to me.
Again - much simplier solution, albeit a rough one - to make their defeat permanent on each difficulty, without future encounters.
P.S. I do understand that there are people around with much more time spent in-game or with better understanding of its mechanics. But higly likely a majority of mod players would be like me - ordinary but seasoned players searching to make a boring game a more diverse one.
Its clearly a normal boss level of danger - to lose a hero no matter the actions, while many bosses allows to, at the very least, set up a fight in your favor or to have a decent preparation.
2 actions per round and 20 protection to all the fuckers is really broken, i liked the jester, but the fucking vestal was the problem
