Version 2.5 (ToME 1.5.0) puts in some safeguards to prevent NPC's from becoming angry at themselves.

This addon may not help with NPC's that are hostile before the addon is installed. Also some actions (like killing a friend) will make some NPCs permanently hostile.

Doesn't seem to work after-the-fact, or retroactively for city NPCs you accidentally hit one time?
