These are only regular models, and can be found in the initial spawnmenu tab under the Addons section like with any other model addon. Just look for either the original addon's name (Starts with the word "Phantom") or the new given name (some big doors[...]). I haven't checked if renaming an addon in the workshop post changes its in-game name, so, check for both to be sure.

As for functionality, I mean, what would you be expecting, really--the doors would have to sink into the ground (which can manually be achieved with something like the Ragdoll Mover addon), and including an animation for the sand in the hourglass falling would have to entail either a 2D animated texture or liberal use of bodygroups, since you can't really set animations on physics props.
