Are you sure you're properly triggering it?
Grab the keycard from the window over the stairs, and scan it to open the door.
If you're desperate, run the Factory Reset batch file in Garry's Mod's main folder.
You will lose all controls, settings, configuration, basically everything except your add-ons.
This'll give you a fresh copy of the game.
But again, this issue isn't with my map - out of the two+ years I've had this up, you're the first out of 53,908 unique subscribers to tell me about this.
I want you to disable every single add-on you have installed, enable Watching Paint Dry, then restart Garry's Mod.
You can do this in Garry's Mod's Addons menu in the main menu.
Click the wrench at the bottom, then click "Disable All".
This will NOT unsubscribe you from all of them. If you're concerned, click "Select All" then "Create Preset" so you can instantly restore your old setup.
If you aren't willing to follow these steps, then I cannot help you anymore

Honestly, you should look into trimming that gigantic list of add-ons. Who knows what each add-on is doing, causing random issues like this to come out of nowhere and break everything.
It's an issue on your end; check your add-ons and disable page after page until you find the culprit.
This map doesn't do anything involving NPCs - all it does is add sounds, and add a few lua fixes for the maps.
Did you uninstall a bunch of maps? Some maps do changes that break zombie behavior.
Try using a default playermodel. If that doesn't work, try disabling every mod you have - you might have something messing with player hull sizes
