Right now you can cut off any part of a prisoner (even their brain!) without anesthesia as casually as cutting a hair.
I have a solution that involves using self-control for roleplaying, so this suggestion is just a bit greedy

why don't you modifiy the Defs so you can achive that?
Alternatively no moodlet debuff and instead add a pain hediff that lasts a very long time and fades with time (like a year or more), to reflect the pawn getting over the trauma, and no special moodlets needed if the negative mood hit is from pain (or positive for those masochists)
This would help force amputation with out medicine as a last resort option, or alternatively a sadistic torture option for prisoners you plan to nugget.

I've sheduled a bunch of operations at once so the pawn was still under anesthetic when their eye was cut out. They still got the "operated without anesthetic" debuff

it's Warcrimes 2, reason:
it adds very simmilar mechanics to "just cut it off"
Side note: it also happens with the "Practice surgery (torso)" operation from War Crimes Expanded, so it's not just this mod.
