Every rare tick of pawn (happens once in ~5 seconds on 3x speed) code excecutes on all pawns assigned to medic worktype if there is available medic work. And this code wakes them up one by one if there is a job that can be done right now. So pawn wakes up, reserves this job and check goes to next pawn. If there are no medic jobs left for him that he can do - he will keep sleeping.
But even if pawn got awakened and there is no job for him - he will go back to sleep. Only emergencies have more priority over sleeping.
TL;DR only on emergencies. Fire or life threatening tending.
Ultimately, the ideal would be for vanilla to have a per-colonist toggle for it, but I do understand why it isn't default behavior.
