As far as the ladder-thing goes, I believe they were having some sort of bug-related difficulty with them. Something to do with the player getting out of sync with the ship when it was moving, or something like that.
However! This is a quote from Marek Rosa, the lead developer for Space Engineers, as of August:
"There were many technical issues with ladders, it’s not a simple item, but I was discussing this with one of my colleagues the other day and he said we should be able to do them properly now because we are learning more and more about the entirety of Space Engineers, so we are definitely smarter now than we were six months ago. Today we are able to do better and without the previous issues. "
So here's hoping!

In response to the question: it was a kind of organic process. I started with a couple basic shapes that I knew I wanted in the design -- the nose, the cross-section of the main hull, the underside of the main engine area -- and sort of developed them until they merged together in a way that looked right. A large part of determining what the main hull was going to be shaped like was determined by the requirements for the cargo containers and conveyors though; so I guess it was designed more from the inside out than the other way around.
Now, if only they'd give us back our ladders ...
