Left 4 Dead 2 is just kind of bad with precaching assets most of the time, I recommend just restarting the game if it occurs and opening the map back up. I've contemplated just packing the assets for my maps in a separate addon to install as I've heard that fixes things sometimes, but I feel as if that's a bit much for a single map.
