yes, i wouldn't blame it on you, ragdolls are very strange at times and do that quite often, its also why some enemies may get up inside the floor, etc etc

huh.. strange, I dont think I ever encountered that issue before. Definitely sounds like a bug with the game itself but I can try my best to see if putting a killzone underneath the whole map can help prevent it but no promises

no, they were totally ragdolled, halfway in the floor, some of their limbs below the floor but their torso and head halfway into it.

Was the ragdoll moving? As if they were stuck in the floor but also like doing a walking animation?

sometimes the ragdolls will glitch out and get stuck halfway into the floor, since the ragdoll is not still they cannot get up. It left me with 1 left, i had to go through the map 7 times and hit every ragdoll once to figure this out.

Some other person said they couldn't find a last enemy, the map has killzones that should kill anyone that somehow gets out of the map.... I feel like it's either the Big Prisoner or the Biker Boss Prisoner that people are somehow missing.
