If you look at some PMCS like blackwater they wear very advances gear but no helmets

why no helmets but the track one does have some/

2 things
1 if they can buy vests like that and neepads and other things like the headgear why can't they but helmet or uniform?

maybe you have some mutators which have possiblity to conflict with multiskins? Idk, that's the only idea I can think of rn

Adding onto what @walkerjt21 said, the multiskins do not change any skins when you enter the game. This applies to all multiskins. Even when a skin of the related group is equipped for both teams, the multiskin does not trigger, and all skins remain the same as they were equipped.

I play on windows, and play on the latest build if that is of any help.
