You can port it for personal use, but I'm currently not giving permission for people to reupload my addons to the Half-Life 2 Workshop as I intend on porting everything over myself and releasing them all together.
Lol, I agree. However this is a dedicated port from EZ:2 and it'll stay that way for authenticity purposes. I do have an alternate version of this addon with some improvements that I use for myself. I may release it soon for people such as yourself.
Hoo boy, that's a bigger question than you realize haha. Depending on the person, it can be a simple process, or it can seem like magic. It took me a long time before I really started understanding how textures work in Source games. Then again, my brain don't work too good.
I'm not going to give you a how-to, because I'll be typing for hours. I will give you some recommendations though. First, get Photoshop or a free program you can edit in. Then download VTFEdit. That's what you'll need for opening and creating Source textures. Then, start researching. The Valve Dev Wiki has a decent tutorial. Gamebanana has good user-created guides [gamebanana.com] as well. If you're more of a video guy, there's plenty available as well.
Why would you want to get rid of it? It's awesome! Just kidding - if you've unsubscribed already, try entering "menu_cleanupgmas" into the console from the main menu. Also restart Steam. If none of that works, then you must have it manually installed. In which case, simply delete it from your "/garrysmod/garrysmod/addons" folder.
If that still doesn't work, then you've missed something and should start over. It's literally impossible for it to be in your game if you've completed this checklist.

It affects performance about the same as the default GMod decals. Which is virtually zero - unless you have literally 4,000 decals on one map. In which case, use the console command "r_cleardecals" to clean up.
