This looks like it could be a version of the Arsonist's Apparatus for the Medic.

This removes most of Medic's team color (his gloves and by extension his medigun), this + a few cosmetics could really screw with people.

I very much like this cosmetic and it's similar style to the one that was made for pyro's foot and arm, though i think this would be even better with the addition of a style between which arm is more robotic, letting you mirror the cosmetic on medic's left arm instead of his right.
