One thing, what is the ending song's name?
I think I only saw one headcrab, early on, but there are a lot of HEVzombs around, in fact they seem to come out of nowhere at the end... is the implication that they are a version of the player who got jumped by that crab, over and over again? If so I absolutely love it. Loved the mythos and the twist that actually, we already *were* in that other dimension, we were not trying to get to it, I suppose we had already made it in the intro, and instead we were trying to get out of that area.
I can't think of anything you could improve for this map, but I want to make a comment that a Gargantua chase could actually be fitting here. That's just a personal note, though.

Awesome map, only criticism is the sound design felt a bit lacking. More ambient metal groaning and concrete cracking would have added a lot.
But I don't think it followed the rules..
