Fun mod nonetheless though.
For the second question, item is in a variety of pools but it has a low drop chance. The item can appear in the boss room, as demon beggar drops, secret and ultra secret rooms and it also appears on every greed room.
First, Is this mod Abandoned? Second, is this item obtainable in game? If so, what Item pool is it in?

The mod menu's default button should be 'L'. Check the workshop page for the mod menu you use if that's not the case.
I'll look into the problem with the upgrade bar. There have been multiple reports about it breaking. I originally thought it was due to a mod conflict.
The visual issue you had happens in large mirror rooms. Unfortunately, it is an old issue that I never managed to fix as I use an extremely hacky way of rendering the sword, and the mirror room really doesn't like it.
And for Explosivo, I might do something for items that make tears stick to enemies. I think 'Sinus Infection' also has a similar issue. I'll look into it.
Is there a way for me to reduce the sfx of the sword and the sword alone?
I was acutely aware that range would be an issue with this item, I didn't really want to add a projectile attack to differentiate it from the spirit sword further.
My solution was to make the charged attack's range longer, which covers a bit more than half the screen width with starting stats. So, I think charged attacks should be able reach those wall creeps.
With that said, I understand that for some cases it feels unintuitive the way charged attack works. Maybe adding a unique projectile attack is not a bad idea after all...
