And your ssj5 version is overwrite by the other mod by kirby7

Why is ssj5 full power so difficult to get

If you have Traits+ and you aren't getting SSJ4, remove Traits+.

Was having problems getting SSJ4 and after removing Traits+, and loading up my game, SSJ4, SSJ4 Full Power and SSJ4 Limit Breaker all loaded.

Can you please make the mod have its own page it has so many forms that it conflicts with others mods. (good mod but it can be annoying that i have to choose between mods)

hm. Others seem to have similar issues but it most likely is a thing with luck, are did you get it without realizing it. because you do not auto transform into it
