We can try starting that background program directly via CMD, then it should print why it won't run.
I'm not sure what can cause this. I didn't test it on that many systems.
Did you try with "Don't lose focus" off?
Did you place your program window on a second screen?
It just boots me out to the desktop, i've got the settings from the second image
I haven't heard otherwise. You'll most likely need to check your logs to see if the background program is running correctly.
The RainWorld window losing focus is a limitation of the Win32 API used. You can try checking the "Don't lose focus" checkbox, which causes you to stay in-game, but your opened program won't automatically close. This does not have any effect if you omit the "open program" option.
With any issues i recommend opening your game's logfile, as it often literally tells you what went wrong.
