Also test without other mods, any mods with broken scripts can make all mods that use scripts break
I can see my item,effects in game but Malektih doesnt get it. Would you be able to help me out ?

It only works if you are empire faction whereas before you could play as high elves and add the ancillary item using the console, can you confirm this is true for me?

I guess my question is how to get the item ID off the mod, for example how to use rpfm or get the item codes off you, so I can manually give my in-game Malekith using console commands such as:
item ancillary_key - where the ancillary key for Dragon Tooth (unique item) is: wh2_dlc13_anc_weapon_runefang_reikland
And so I could add the item via the console using this command:
item wh2_dlc13_anc_weapon_runefang_reikland
If you could help I would love to use the items you have created :p
