Weird, never experienced that personally but I have seen a similar issue with the shotguns. Could be a load order issue, but i'm not too sure. Do you have another mod that also replaces the L4D2 pistols as well or just this one?

SIG and Glock show as M1911s in L4D2 campaigns when they're on the floor or in other survivor's hands. Is there any way to fix that?

Yes. This is because the original mod uses the original Left 4 Dead pistol icons, which did not include a flashlight.
