Report a translation problem
If you were too exhausted or fatigued for the last push, it will say "nooo..."
If you were just unlucky ("Use Chance" not passed), it will say "..."
If no text appeared above the character's head, then it's probably a bug.
Can you share the exact Last Push Trait sandbox settings used?
Short answer: automatically when you are overwhelmed.
By default, you earn the trait at level 10 strength.
You can change the strength level or disable earning the trait in "Last Push Trait" sandbox settings tab.
I never tried it in multiplayer, should've added a warning about it. Added it now.
No, the amount of zombies pushed is currently unlimited - all in configured radius will be pushed.

Mods like this that ease back the hardcore permadeath threat the devs saddle everyone with while also keeping things balanced and sensible are a godsend to players like me who still want a challenge, but don't want to lose weeks worth of playtime cuz some stupid fence lunger knocked you TOWARDS it somehow and into a bunch of zombies waiting to instakill you. All that to say, thank you! For sure using this on my next character.

It's automatic.
It's activated when you are getting stuck into the "drag down" animation which would normally kill you without any chance. This is a vanilla sandbox setting "Drag Down".
There is also a hit activation provided as a fallback when you have "Drag Down" disabled. Basically, if Hit Activation is enabled, the ability will also trigger when you get hit by a zombie and you are surrounded by them. You can control exactly how it works in Last Push Trait sandbox settings. See more details in settings tooltips.
It is only required if you have Drag Down vanilla setting disabled. You can change any settings of this mod without the need to restart the game/save, they will apply immediately.
I recommend using Change Sandbox Options (by Star) for changing sandbox options at any point in the game.
I think in future versions this setting will be disabled by default, and enabled automatically if Drag Down is disabled (Because otherwise there will be no way for the ability to activate).
Should probably include the explanation of how it works in the workshop description as well.
For now I'm pretty sure it's explained well in the sandbox tooltips - just hover over the settings to read more about them. Let me know if any of the tooltips are confusing and I'll try to correct them.
There is no delay currently, it activates immediately both when animation is triggered and when hit.
I'll add the configurable delay for both activations when I have time for this... Which might be a long time from now.

The bite is not guaranteed, only a hit is. If you got bitten... welp.
I have no idea how to prevent a bite codewise. I haven't seen any mod that does it. If you have, please tell me - I study the code and implement it.

In my tests with default Push Distance setting the delay was just enough to be able to escape from fast shamblers. You can increase the setting if you feel like there is not enough room after the last push.
I recommend using Change Sandbox Options (by Star) for changing sandbox options at any point in the game.
Also, the stagger is kinda required by the game - it interrupts the death drag down animation. I haven't found another way to stop it.
I reduced it to 10 ingame minutes to avoid further confusion.
I'm sorry for that . Thank you for reporting the bug.
Fixed this in the newest update, as well as replaced the "Kill Instead" setting with more interesting damage settings.
You also need either Drag Down (Zombie Lore tab) or Hit Activation (Last Push Trait tab) enabled for the ability to have a way to activate.
