It isn't a playermodel. Its a cosmetic you need to to bonemerge onto Scout's Ragdoll wtih the help of Easy Bonemerge Tool.

The Head is a stand-alone cosmetic. The creators didn't make any Arm or Leg Cosmetics to go along with it.
