Boogie-Woogie - A passive item. Double-tap to fire a projectile that marks an enemy. Double-tap again to switch places with that enemy. Possibly better suited to an active item.
Malevolent Shrine - Much like Infinite Void, for the next two rooms the player has a large aura that does AoE tick damage to enemies caught in it, lasting for only a few seconds per room, and deals a half-heart of damage upon use. Alternatively remove the half-heart damage and have the size/tick damage/player's Speed increase passively with every use.
Prison Realm Active item, using it places it on the ground and after five seconds every enemy within range is trapped. Using it again releases the enemies, leaving them Confused for a set time, and it must be empty to trap more enemies. Affects bosses maybe.
Heavenly Pact - Similarly to Abyss this active item consumes all pedestal items in the room, converting them into pickups ala Tainted Cain. Potentially could be better suited to a Card/Rune type item.
Maximum Uzumaki - An active item that requires picking up a rapidly disappearing "soul" resource from slain enemies to charge. It will fire a homing explosive shot upon use, the amount of "souls" picked up equal to the power of the projectile. Must be held to charge ala Breath of Life, with a shorter charge time for a weaker projectile.
Lil' Mahoraga - A familiar that orbits at a similar distance to Distant Admiration. It fires tears of whatever type last hit it, inheriting their effects.
