Update: The problem model seems to be "models/objects_interior/flag_02.mdl"

I have no idea why it's breaking the spawn icon renderer...

I seem to be having issues with some of the models, this particular addon seems to be overriding the crate HP (according to Addon Finder) and at least one of the included models crashes the game when trying to render its spawn icon.
