My only nitpick: Why a knitted argyle sweater/pullover under the coat? Why not a normal shirt in red/blue?
The typical Wild West town in movies is located in a desert or on the Great Plains prairies where it's pretty warm. Not a climate for woolen sweaters.

Cool idea. I could totally see one of the Medic's ancestors being a travelling barber surgeon in the Wild West, who *seems* like a snake-oil saleman but the twist is his miracle cures work (if you don't mind the side effects).
