The other mod I have that touches prefixes is the sorbet cafe prefix mod, and then the spirit mod uses glyphs which are kind of used like prefixes
Maybe the magic storage I'm using is so full of items that it breaks with this mod? I'm just guessing
Maybe I can add you over discord or something and send you my world file and modlist? If you're busy I totally get it, I wanna help fix out the problems of this mod because its so cool

my best guess is that there's too much data being stored in the weapons and that makes magic storage act up since it isnt expecting it

I'm trying to take a bee gun out of magic storage and it remains stuck, the server log throws that error. I'm not sure if this is a problem on your mod, or magic storage

If you use the regular reforge option, these won't get wiped should you roll onto the two prefix option.
It was off hand slot, actually. For some reason your mod attaches itself to the offhand slot and not the actual reforge slot. Off hand slot isn't needed, i like yours more. So i'll be deleting my other comments. Sorry again about not troubleshooting before commenting </3
