Admittedly it's ugly jank to get this to work in the first place, it's going to take a lot of wrangling with unintended behavior and code to actually implement this into a mutation and patch issues it already has.
You can just restart L4D2 when you complete a campaign, which should reset all the saved deaths.
It's probably one of the mods that are in yoru's nightmare collection. It's the pack that I was using for that video.

Is it still causing issues for you? I tried it without mods and with your modpack and it works.

Weird, the mod works fine for me. Check if the "PERMADEATH VSCRIPT HAS STARTED" message plays in the console, otherwise I really can't do anything else. You can try the original mutation and see if that works for you.
It's likely a mod you have installed that's conflicting with the script. I'd recommend disabling all of your mods and then gradually re-enable them in segments until you find the mod that's causing the problem.

Yeah I can do that. I'm assuming you don't want every survivor to respawn when all 4 die right? Survivors that started as dead will still be dead after the round restarts.
