Before 1950's people were using pistols/revolvers with one hand actually but during mid 50's and early 60's People started using Pistol/Revolver with two hand due to being more comfortable however some people was already using Pistols/Revolver with two hand before 1950's

Idk why people think firing one handed with a revolver is painful. I've always found it more comfortable for dampening recoil, something you don't have to do on an automatic since their slide functions to do that.

Yeah they did forgot speedloader too.

They forgot (or removed?) speedloader mesh, although the bone is here and fully animated

No, Valve forgot to add left hand to AR2 so you're holding it with only right hand and Gravity gun you also hold it with left hand. Same goes for Crossbow as well if you look up with viewmodel you'll see Gordon holding Crossbow with only left hand but he doesn't have right hand to hold the trigger.
