He said its an addon conflict on discord. I also checked. This scenario only occurs when this addon is activated, as this does not occur with your other addons (HL:A, +CUPE+ Plus, Anomaly).

Here to report a big bug. Combine Suppressors in the original Combine Units +PLUS+ will immediately go in an injured animation and lose his weapon upon spawning back. Proof: https://streamable.com/qegcjg
