the reason I put forsaken in the title is because i made it based on his forsaken sword slash version, with the cooldown the same damage and the fact it stuns

did you check whether or not its on cooldown?
did you try with all mods besides this one disabled?
if you were able to get the weapon in weapons section in your spawnmenu, did it slash? and if it did slash it could just be because you have bad aim or bad timing.
