Klei implemented the Caves a long time ago. You need to have dedicated server for Caves or use the mod "Advanced World Generation" with "Infuse Worlds" option enabled. Or change the recipe to craft Thulecite right away, instead of Fragments.
It's not possible to add 2 recipes that are different, but create the same item. Currently original Caves have been introduced to the game and they always spawn with Ancient Altar which you can find and combine fragments into Thulecite. If you want, you can always enable the recipe to craft Thulecite right away, instead of fragments, this is why there is such an option.
I add all the updates done to "Change Notes". Can read them there. Basically i optimised the code and moved the altar crafting under Magic Tab etc.
You have to change it from mods options on which recipe you like to use, either Thulecite or Thulecite Fragments.
Cave Content mod does include Ancient Altar, but beware, it can be destroyed like any other structure. So if you don't have any mod that makes it a respawnable building, you should have this mod so you can always rebuild it. To make things renewable, i would recommend "Fix Multiplayer" mod for that, the author for that has added in Cave content stuff so it works very well and if the Altar gets destroyed, it gets added back after some time.
Also, when you generate world with Cave Content mod, note that not always the ancient altar gets placed or Pig King for that matter. It's world generation problem in-game, not the mods problem, so simply regenerate a new world and make sure Pig King and Altar etc are there.

If you want Cave content, try a mod called "Extended Map Generation (Cave Content)" and for respawning things a mod which is compatible with Extended World Generation mod as well "Fix Multiplayer".
Altar itself is under Structures Tab, Thulecite is under Refine Tab. I could think of moving it under Magic Tab, perhaps it makes more sense, we'll see.
I'm assuming you mean Thulecite Recipe? If so, then it is under Refine Tab, the screenshot is outdated.

Because i just tried to test and it seems working fine on my end i can't reproduce the bug at all.

Someone else reported a similar bug-report and i still couldn't reproduce the bug. I did however push an experimental update that either breaks everything or fixes something... try it out and let me know. Also, can you tell me what exactly was that you did when the bug occured?

You can craft the Thulecite with the help of this mod as the mod name says "Craftable Ancient altar AND Thulecite"
Try the mod now, i updated it and hopefully the bugs are gone.
Are you using any other mods in your server?
Because the mod works just fine and does not crash the server when cave creatures spawn, it shouldn't be able to crash it, because the cave creatures exist in the current game anyway, some are just not enabled yet.
I fear it's some other mod conflicting with it somehow.
