It's not an addon, it's a dupe. You will have to spawn it in your game through Dupes.

To think that someone took their time to make Tetris within Garry's Mod gives me back my lost faith in the Garry's Mod community and humanity. FNaF won't take over after all ^^

You should add sound system to that, the system itself would be disabled on user demand.
