Those are REALLY good ideas!
Israel could start with no settlers, but a stronger military made OP by the unique trait (which would only last temporarily), as Israel conquered its cities, they did not settle them.
Another name for the trait could be "Promised Land".
And there seems to be a bug: I was not able to spread religion with the Ark of the Covenant. Neither own cities nor cities of the enemy (neither durig war nor in peace). And think about more movement in neutral or own area. The next is - I use it nearly only as citadel. So for me is the general better. Set the citadel next to a city - the stationed unit always gets damaged.

Free worker with new city exists already - nearly (with built monumet replacement you get one - Armenian Empire). So I like this idea more.
UA - I fully agree. A disadvantage as UA - what's this???

You have GREAT ideas. If only I was modder enough to make them come to life.
Sadly I have no clue how it would be implimented... How the player would know which cities are on the promised land? A pop up maybe? And can you limit your UA to only a few other cities? (maybe the first 3 cities found are the promised land)
To answer a majority of your questions, I heartily recommend reading the Book of Joshua in the bible. It will all make sense.
Summary
+25% vs. city states ( Joshua and the israelites clove a bloody path through many city states while climing the land.)
-25% relations (Gotta balance that previous, because I had more coming. Also, City states indeed despised the israelites coming to kick them out. Thats a fact, jack)
-No Gold Plunder (See Jericho/AI city state capture in the book of Joshua. God forbade any plunder, Some guy took plunder anyway, God kicked israels ass for it.)
-Faith on kill (used to add a positive to the afformentioned negative, and to replace Gold with Faith in a way.. Had to stop it at midevial era to prevent OP allegations.
So basically, All of the UA things you're questioning have a grounding basis in biblical history. Thats why.

I don't like the whole 25% increase combat strength vs city states. That's mongolia's thing.
Why do city relations decline 25% faster? (sounds like a bandaid nerf)
Why don't they gain gold plunder? (sounds like a bandaid nerf)
Why do they stop gaining faith from kills after the medieval era? Shouldnt it stay all the time, just as a flat value so it's useful early game but doesnt affect much late game. Or you could reduce it's power based on the era you are in. (similar to the increased faith costs for non great person purchases)
Also, small thing. I feel like "Chosen people" would be a better name than "God is with us".
Why do isrealites build workers faster? i mean it's a cool mechanic sure, but i don't recall hebrew people being amazing builders. Instead wouldn't it make more sense to start off with a hebrew worker, as they were slaves for a long time? Or maybe when you settle a city you get a free worker with it. They would definately have to be much slower at building things for balance reasons, but i feel it would at least make sense.
UU - Arch of the Covenant
I LOVE THIS. It sounds amazing. Only thing i don't quite feel 100% about is the prophet spreading. BUUT other than that it sounds really, super cool.
and woah, random thought. what it settlers gain +1 vision, and give faith per turn because they're looking for the promised land. (stops past the ancient era to prevent abuse)
Overall i do like the idea of the civ. I just feel theres a few things that don't quite fit or are unnecessary.
Sulu, Im afraid I doont have enough info to diagnose that problem. It sounds quite local. I have the issue occasionally, where it wont appear on my mods list... It comes and goes. :(

Canaan i hesitate, because that is essentially phonecia (I.E, Carthage. Dido.)
Which is a shame as they are like, one of the most well supported historically.
