As stated in a previous comment, follow these steps. Please keep in mind that you may have to create the "custom" folder to place the mod inside.

Navigate to ...\Steam\steamapps\workshop\content\222880\596539619\ in your local files.
Copy the Military UI .vpk file.
Paste the file in ...\Steam\steamapps\common\insurgency2\insurgency\custom\

Due to updates and patches, the modded main menu does not appear on startup, but will appear when you return from a game.

I did not have a \custom\ folder in \insurgency\ so I just pasted it directly into \insurgency\ along with the other .vpk files.

It should work after these steps, since file pathing in the mod has not changed since the original upload.
