Here is examples of some I saw, not all of them of course:

You will notice them more with a white flag background or one color flag background with no design on it.
I'm in the dark about what did the trick; the hand-crafted .mod file was no different from the previous attempt since I re-used the previous one...
Anyway!
Managed to cobble one together from another mod and get the emblems to show up in-game, but the launcher keeps claiming it can't find the archive, so I must have messed up making the .mod file.
I tried to manually create it using another ucg_ file as a template. After doing so, when starting the Launcher the mod then does show up, but with an error message that the path to the archive doesn't exist or is invalid.
I used this line /path:
archive="C:/Program Files x86)/Steam/steamapps/workshop/content/281990/686093209/extendedemblems.zip"
I confirmed that he path is correct, except that there is no .zip at that location. However, the ucg files of all the Stellaris mods I have installed refer to a .zip file, even though none of them have actually one there. Maybe I got the extendedemblems wrong?
Other than that, this seems like a Steam client thing?
I have no idea what it could be yet. I tried several scenarios with unsubscribing and subscribing and having the Launcher open or not while doing so, but was not able to fix it yet. Working on more drastic measures; will keep you posted...
Anyone else having this problem?
I do remember something weird with people using UI Overhaul Dynamic and Planet Diversity, if they use both mods, it won't let you scroll the planet menu, so might be for the whole menu. This is fixed if players use the compatibility patch for UI Overhaul Dynamic and Planet Diversity:

Other then the mod descriptor needing right version number, this mod is perfectly fine and should lose the [Outdated] title.
Hope this helps.
If you need anymore info, just message me in Steam.
If you do not have Stellaris installed anymore, you can download this mod and check it out.

Just put this in field:

I have not had an issues, I can scroll only because I UI Overhaul Dynamic.
I will double check on my end, but mod seems fine.
I want to say that I love this mod and I'm going to try it out to see if there is a perfect match for my Future-Tec Incorporated empire that I will use for when the MegaCorp expansion comes out for Stellaris alongside the 2.2 update. Thank you very much for this mod. :)
