The best method I know is to go to your steam installation folder and follow this path
\steamapps\workshop\content\440\786267802
The map should be located there, copy it over to your tf2 maps folder
\steamapps\common\Team Fortress 2\tf\maps
Then in console you type "map tr_monoculus_b2"
Valve just didn't bother to make it work any simpler.

@noahrhag, I think you get everything except the bombinomicon, which you would get when you get it in the island in eyeaduct.
