I like the reference but it does seem a bit at odds with the sprite for the item.
(Oh, and trinkets and passives overlapping functionality is as old as WotL, so don't worry about it. (Infestation and fish head, and pinky eye and common cold.)

You can turn your health back to normal though which you cannot do with missing poster with most characters once you're the lost

The item itself doesnt really look like the telltale heart from dont starve though, there it is some spider guts bound together with rope.
Especially the visible Lost health, really creative addition.
