if u have only one one-time-usage(OTU from now), then, after using, item isn't deletes as should, but staying in second slot, allowing to use it infinite time.
same with chargable items and "butter" trinket, they even not discharged.

If I were you I would drop the item in the room just so the item isn't lost forever if the player loses the backpack and they can then pick whichever item they want to continue with.
That is true, but come on. They could've at least made it something else, like with what the Off-Hand did. Making it a Backpack, like Antibirth's, is kinda stealing it.
Then again, that just might be the idea everyone had, make a backpack item that allows two actives.
