Otherwise go to the Steam Library, on the entry for TKOM click 'Play' and then 'Open Launcher' then left click 'Reset Game Settings to Default'.

Yeah... I just checked it. You double-click with the mouse on whichever one you want to change and then just click which button or hit a key on the keyboard to change it to that key. Enter will only change the keybind that is currently highlighted.

Unless you've right clicked (which stops the camera movement with mouse and lets you only move the cursor for clicking things like the refinery's buttons) then the camera should be moving along with the mouse movement.
You might want to check the drivers for the mouse or it's settings in general. Also if you have any other controllers like joysticks, driving wheels or the like try unplugging them.

You've either screwed up the settings thinking you needed to re-assign keys, something isn't set right with the mouse, or as I stated drivers and unplugging any other controllers you may have plugged in. This is NOT KSP or any other game... apples and oranges.
