AFAIK slots themselves are hardcoded, but items can be made to use e.g. a headset slot (which is otherwise useless legacy element) and that can work seamlessly if headset functionality is added to them. Same with belt slot.

Xpath-Lua can insert these changes into existing items from other mods.
