Mods, created with the in game editor, are located in C:\Users\[USERNAME]\AppData\Local\KillHouseGames\ActionSquad\mods
But, as wrotiien in the linked guide, for the upload on Steam Workshop:
"When you're ready to upload your level to the Steam Workshop just select the Workshop menu again and click the Mod Uploader option. Fill in the required data, select an image and click Upload. That should get your level up on the Workshop."
You created the mod... You placed the graphics...
What happen

Please open the log file located in C:\Users\[USERNAME]\AppData\Local\KillHouseGames\ActionSquad and see the path about loaded media are located and move them to the mod's folder and modify the mod accordingly.
