SS3: Extended is more like an unofficial patch for the game, so it is similar to the original, just with changes that in my opinion of that time made the game better.
BFE Enhanced is more like a mod: It changes a lot more than just Extended, including enemy spawn and how the levels look and play in general. From all 3, this offers the most different experience.
Actually, Extended was modified to be like that as well, though not working completely well. For some reason, having Enhanced on disables the in-level quotes from Sam, and I dunno why, as they are controlled by newly resaved scripts now. But besides that, it doesn't have any conflicts with other mods whatsoever anymore.
The only problem with the above video is that the level showcased from Extended wasn't changed that much compared to some of the others. :P
