I'd like to suggest the addition of events to force/ask you to appoint a co-emperor. For example, your wife asks you to appoint her favorite son/herself/a lover/an influential family member as symbasileus, or your mother asking you to appoint a brother of yours for example. On the other hand, if your wife, mother is symbasilissa and ambitious, she could attempt to overthrow you. (Source: https://en.wikipedia.org/wiki/Constantine_VI)

If you have traits such as imbecile, slow, incapable, weak, coronated basileus or not being coronated, bedridden (if possible) and such makes it more probable for the event to appear, or even unlocks choices that forces you to accept.
