EDIT: Could it be that the mod isn't COW compatible? I know that there is a separate Imperial Government mod and that wasn't compatible with the COW mod.
I've found out that it isn't COW compatible. This mod does not change normal governments, but COW does since it implements a new holding type which requires a change in how all other governments (feudal, republic, theocracy) view it.
So, by using both mods, COW won't tell the Imperial Autocracy government that the new holding type is "viable" for it - so that might cause a few problems.
I haven't tested this out, but you should be able to make them compatible by going to the government\RHOMAIOI_government_type.txt file and manually inserting the name of the holding COW implements.
Thanks for the clarification, I think I've managed to correct the mistake and am uploading a new patch. If you encounter it again in a new game, please do make sure to tell me.

Could you please provide more details? Does this happen always?
Note; This effected the AI as well.
Should be fixed by now, was an issue where the Pope could opt for the Byzantine succession system (maybe it's just the Pope trying to get back under the Empire before they get there and kick his but!).
