Settlement disappeared after raids
I started my game with an allied settlement nearby. At some point it got raided and captured by a hostile faction. Some time later the captured settlement must have been raided again, because now it's gone.

I thought raids usually resulted in a settlement being converted and under the control of the raiding faction, not just disappearing. Under which circumstances does an AI-controlled settlement disappear?

Settlements can be destroyed or captured. If a settlement is defeated when having fewer than 1k points, then it will always be destroyed. The message will appear as "pathetic hamlet was burned to the ground." If the settlement has more than 1k points then there are other factors that play into the results, such as if the attacker behavior and how many points the attacker beat the defender.

There's also a bug that can inadvertently destroy a settlement if a scout on the map has the settlement as a target but is then engaged (by another unit) on the map. This bug triggers the combat results as if the scout had defeated the settlement. This bug will be fixed in the next update.
