Edit: Just upgraded to lvl3. Were flashlight upgrades always lvl3? I guess I'm so used to attaching them to my belt that I've just never used the flashlight backpack slot.

I narrowed it down to the Item Condition mod. I had removed several mods beforehand notably the Reorder the Hotbar mod which seemed like a potential conflict as well. After Item Condition was gone I was able to attach things to my bag normally.
I verified this by saving with a bottle attached to my bag and trying to re enable the Item Condition mod. Game crashed immediately. I then re added the other mods I had tested with beforehand, and everything is working smoothly. Whether this is a condition of my extensive mod list or an overall incompatibility I don't know
Also any time you add or remove a mod to your game it is good practice to hit the mod order button next to accept and re sort your mod load order before continuing.
I have a L1 upgraded duffle bag with a bottle slot, but Gym Bottle, plain Water Bottle, and Coke Can all are NOT able to be added to the Bottle slot. I can't find anything that will go into the bottle slot actually.
I'm only running AuthenticZ the base mod that included both the outfits and the backpack upgrades.
