The cities, the armies would be affected by the diseases. A disease spreading system.
Possible loss of populations, economic loss, unrest of the population.

Characters could be affected by diseases. (Generals of infected armies, governors if regional capitals are infected.)
The Vulnerarius could play a bigger role.

New decisions could emerge to try to contain epidemics.
