You should not be getting these as Tainted Forgotten to begin with, no. That second part sounds even weirder. You keep getting soul hearts back all the time after room clear, even when not taking damage?
The goal is to just make Immortal Hearts work "normally" for The Soul, but the modding API of this game is certainly not making it an easy task, so they should have been temporarily disabled on The Forgotten. I'll give it another look.

No, this mod does not override any files, only add them.
Are you by any chance playing as The Forgotten or the Keeper, or picking them up with Esau? Perhaps playing coop? Because they work for Esau but don't show up on the HUD yet. They have some issues with Tainted Jacob when you go into the Lost form, and the Forgotten/Keeper cannot have them at all (that includes the Soul).
EDIT: downloaded mod config, hud is fine now
1- if you have exactly half a soul heart and you pick up an eternal heart, it will heal you for half a soul heart. This is also applying to black hearts. Example: https://gyazo.com/633d6a108405146e7bcd210a6f2140d8
2- in Downpour/Dross II, if you enter the mirror world, the icon for the eternal heart does not become faded like the other health icons do. Example: https://i.imgur.com/P3L6xVO.png
The second part is something I will try to fix, but no guarantees. The way I have to render the hearts is actually pretty hacky, because otherwise I can't render them over the HUD. Chances are I'll just let them render as soul hearts when in the mirror world, since they shouldn't be applicable there anyway.
I also fixed a few other rendering glitches and possible black screen bugs. I'm still working on fading out the heart display in the mirror world and pause menu, so bear with me. I'll try to knock those out at the same time as proper HUD compatibility with The Forgotten and Esau.
Displaying the hearts for Esau will probably happen, but coop immortal heart display probably won't (they'll just show up as soul hearts for P2). The health display is totally out of wack in coop, and gets even harder to predict when people play as Jacob & Esau.
This issue should probably be fixed now. Let me know if you still have any issues.
I just fixed it (for real this time!).
It looks like this was a bug in the modding API, which I was able to work around for now. The mod should auto-update the next time you start the game.
