What you mentioned could be what happened. I think it is technically possible, but only if one of the projectiles somehow hits an exception trying to deal damage
I haven't been able to reproduce this locally (well intentionally anyway), but I pushed an update to fix the incredibly rare edge case above. Maybe it'll help, feel free to let me know if you run into this again
