I don't know if this is a bug, but I just managed to Game Over myself by sending all three of my colonists (I'd just started the playthough) into a caravan, and then set up an outpost to rest for the night. I got the Game Over alert, which wouldn't go away even after I reformed the caravan.

I'm running version 1.5 of the game and this mod, if that information helps.
