I figured out the reason why Central chided me for missing the list from the executive terminal earlier; Lily had the site list in her inventory and when I clicked the teleport button, only she teleported out. I had to click again to whisk Gana away, but at that point none of the present agents had a list, so I got reprimanded.

An unrelated (and much more beneficial) glitch seems to happen with Lily's free-peek ability during alarm level 0. If you have 0,5 or 0 AP left and you use Lilly's free peek, your AP returns to 1. You can then move a square and repeat this as long there is something to peek around/through. This won't work once the alarm level hits 1.
The other one might be an incompatibility with Community Bugfixes but I'll get back to you on that.
Thanks for reporting the AP thing, that IS a genuine bug/oversight. ^^; What happened is that instead of making Lily's peek free under alarm level 1, it's actually easier to lift the restrictions for it, then refund the cost... but using AP can't take you into negative AP, therein the exploit, oops! Enjoy it while it lasts, I'll fix it ASAP.
Thanks for looking into it. I did some more checking and I don't think either More Archived Agents or Community Bugfixes is the cause because I removed both of those mods from my mod folder and the glitch still appeared. (in fact, the glitch consistently turned up when I tried to play with only this mod, the Sim Constructor and Function Library installed)
The teleport glitch definitely seems linked to Lily. Whenever I played a level after picking her, teleporting out always glitched. I didn't get that issue if she wasn't on my team.
The glitch seems harmless at first since you can leave the level by either clicking the "exit level" icon on the agent whose teleportation failed and usually whenever you're chided by Central for failing an objective, you don't actually fail. (you still get locations from the executive terminal even if the agent who held the site list teleported out first and agents you rescued still join your team)
It can mess up your mission if you play the More Mission Mod and the courier you rescue doesn't teleport out last. (if you try to teleport and he phases out while one of your agents fails on the first try, you don't get the new locations)
I found that it outright breaks the game on the last mission. Central entering the final chamber is supposed to despawn all other playable characters, but they fail to do so and you can't plug Incognita into the mainframe, meaning you get stuck and there's no way to finish the level.
Try the following:
1) Pick Lily and another agent
2) Have Lily get the site list
3) Put both agents in the exit teleporter, switch to Lily and click her "exit level" icon.
She should disappear and either the other agent gets left behind until you click his/her exit-icon or both agents teleport out but the level doesn't end until you click the end turn button.
This happened to me every single time.
